India Now a Leading Destination for Haploidentical BMT — Why It Matters for International Patients

One of the biggest barriers to bone marrow transplant (BMT) for many patients worldwide has been finding a HLA-matched donor. With smaller families in many countries, the chance of having a full HLA-matched sibling is often only 20–30%. India’s top BMT centres have now made haploidentical BMT — using a half-matched family donor — a routine, established treatment option, opening BMT access to a far larger pool of patients.

What is Haploidentical BMT?

A haploidentical (or “haplo”) transplant uses a donor who is a half-match — typically a parent, child or sibling. Until about a decade ago, only fully matched sibling donors were considered safe for allogeneic BMT. Advances in conditioning regimens (particularly post-transplant cyclophosphamide) have made half-matched transplants nearly as effective as fully matched, with manageable risks.

Why It’s a Big Deal for International Patients

Many patients with leukaemia, lymphoma, multiple myeloma, thalassaemia, sickle cell disease or severe aplastic anaemia cannot find a matched sibling donor. Without haploidentical BMT, the only options are:

  • Wait years for a matched unrelated donor (MUD) from a registry — and registry matches are rare for non-European ethnicities
  • Cord blood transplant (limited cell dose, especially for adults)
  • No transplant at all

Haploidentical BMT solves this by allowing virtually any parent, child or sibling to be a donor — meaning most patients have at least one possible donor in their family.
